Enteropathic Arthritis

Definition

  • Inflammatory arthropathy associated with chronic IBD
    • Crohn's
    • UC
  • Peripheral arthropathy in 15% of IBD
  • Spondyloarthropathy (sacroiliitis) in 5% IBD

Clinical Features

  • Peripheral Arthritis
    • Acute migratory attacks ~ 1x / year
    • Lasts < 1 month
    • Associated with flare of IBD
    • Affects large joints
    • Transient
    • Non-deforming
    • Synovitis only
  • Spine
    • Classic AS
    • Persists if present
        • Treatment of IBD will not get it to settle

Differential Diagnosis

  • AVN hip due to steroids
  • Fistulae to Psoas or Hip

Treatment

  • Treatment of IBD causes arthritis to abate
    • Sulphasalazine effective
  • Spondyloarthropathy treatment as for AS
