Enteropathic Arthritis

Definition

Inflammatory arthropathy associated with chronic IBD
- Crohn's
- UC
Peripheral arthropathy in 15% of IBD
Spondyloarthropathy (sacroiliitis) in 5% IBD

Clinical Features

Peripheral Arthritis
- Acute migratory attacks ~ 1x / year
- Lasts < 1 month
- Associated with flare of IBD
- Affects large joints
- Transient
- Non-deforming
- Synovitis only
Spine
- Classic AS
- Persists if present

Treatment of IBD will not get it to settle

Differential Diagnosis

AVN hip due to steroids
Fistulae to Psoas or Hip

Treatment

Treatment of IBD causes arthritis to abate
- Sulphasalazine effective
