ALL questions must be answered - Question I (50%) & Questions 2 & 3 (25% each)

Question 1 (One Hour)

A 26 year old professional rugby player sustains a severe varus injury to his right knee 2 days previously. The team doctor interprets the injury as a complete rupture of the lateral ligament and posterolateral complex with intact cruciate ligaments and a complete foot drop.

  1. Write notes on the clinical assessment and investigation of the injury.(30 marks)
  2. Discuss management of the lateral ligament and posterolateral complex knee instability, and include details of surgery.(40 marks)
  3. Discuss management of the foot drop.(30 marks)

Question 2 (30 minutes)

An 80 year old man with a 20 year history of severely debilitating chronic rheumatoid arthritis presents with a 6 weeks history of recurrent collapse / weakness of the legs. This is threatening his independence at a special accommodation village.

  1. Outline your clinical assessment of this gentleman.(30 marks)
  2. Outline the risks of surgery for rheumatoid arthritic afflictions of the cervical spine.(20 marks)

Question 3 (30 minutes)

A three month old, otherwise normal first born-girl, presents with a previously undiagnosed dislocation of the left hip.

  1. Outline: Your initial assessment (15 marks)
  2. Your treatment of this child (15 marks)
  3. The issues you would discuss with the parents (10 marks)
  4. Describe:The anatomical pathology involved and its significance (10 marks)
