  • Medial dislocation of Biceps tendon from the groove

Aetiology

  • Attrition rupture of transverse humeral ligament
    • ± Associated rotator cuff tear
  • Acute traumatic rupture of transverse humeral ligament

2 Subluxation at the groove
3 Malunion / Nonunion of Lesser Tuberosity

History

  • Pain in anterior shoulder
  • Dislocation / Subluxation of Biceps Tendon with abduction & external rotation
  • Snapping sensation
  • Reflex dropping of arm with subsequent relocation of tendon

Examination

  • tenderness in groove
  • Attempt to dislocate tendon
  • Pain & pop with Yerguson’s test in adduction

Investigation

  • MRI
  • Arthrography

Differential Diagnosis

  • Recurrent anterior GHJ dislocation (same position of abduction with ER)
