Aetiology

Attrition rupture of transverse humeral ligament
- ± Associated rotator cuff tear
Acute traumatic rupture of transverse humeral ligament

History

Pain in anterior shoulder
Dislocation / Subluxation of Biceps Tendon with abduction & external rotation
Snapping sensation
Reflex dropping of arm with subsequent relocation of tendon

Examination

tenderness in groove
Attempt to dislocate tendon
Pain & pop with Yerguson’s test in adduction

Investigation

MRI
Arthrography

Differential Diagnosis

Recurrent anterior GHJ dislocation (same position of abduction with ER)

Treatment

Tenodesis if troublesome
